Congratulations and welcome to the Safe Haven Senior Living team. In order to get you into our system we need you to do the following.

• Download Safe Haven employee packet by CLICKING HERE
• Download W4 Form by CLICKING HERE
• Download Separation Notices by CLICKING HERE
• Print out both packets in its entirety.
• Read the entire packet very carefully as you will be tested later on.
• Sign where appropriate and initial every page of both packets.
• Bring both packets on your first day of work. Must be completely filled out prior to you starting work.
• Don’t forget you MUST bring your background check, CPR and First Aid, Resume, Physical and TB Screen with this packet along with W4 Form, W9 Form, and Separation Notices. No Exceptions!
